About this role
Overview
NYREACH is a Bronx-based non-profit focused on addressing health disparities through community-driven solutions. Our work connects individuals to healthcare and essential social services while building strong partnerships across New York City.
Job Summary
The Project Manager will coordinate and manage the many projects running across NYREACH at the same time, including the Social Care Network (SCN) program, the Medicaid Recertification team, Workforce Development, and other core programs. This role keeps work on schedule, makes sure every project has a clear owner and next step, and gives leadership an accurate picture of where things stand.
NYREACH runs multiple projects at once, many tied to health plan contracts, state waiver requirements, and fixed deadlines. The Project Manager owns the master project plan, runs regular check-ins with team leads, and follows up until action items are closed. This is a coordination role: the Project Manager does not supervise the navigator field team or lead business development, but works side by side with the Managers, the Compliance & Data Analyst, and program staff who do.
Responsibilities
Project Portfolio Management
· Build and maintain NYREACH’s master project plan, including timelines, owners, dependencies, and status for every active project
· Run weekly check-ins with team leads and follow up on action items until they are closed
· Identify schedule risks, staffing conflicts, and blockers early and bring them to the Director with recommended solutions
· Help leadership set priorities when projects compete for the same staff, budget, or deadlines
· Set up simple, consistent project tools and templates (trackers, timelines, meeting notes) that teams can use day to day
Social Care Network (SCN) Projects
· Coordinate SCN workstreams such as screening targets, enhanced services delivery, billing cycles, and health plan deliverables
· Track deadlines tied to health plan projects, scopes of work, and Lead SCN requirements
· Support the rollout of new or updated SCN workflows, working with the Compliance & Data Analyst to make sure process changes are documented and communicated to staff
· Coordinate logistics for audits and health plan reviews, including timelines, document requests, and follow-up items
Medicaid Recertification Team
· Plan and track recertification outreach campaigns, including member lists, staff assignments, and weekly renewal targets
· Monitor progress against application and renewal goals and work with team leads to adjust plans when numbers fall behind
· Lead project planning for upcoming program changes, including six-month renewal cycles and Community Engagement requirements under H.R. 1 (effective January 2027)
· Track staff onboarding milestones, such as NYSOH Assistor/Navigator certification within 30 days of hire
Workforce Development & Other Core Programs
· Support Workforce Development projects as needed, including scheduling, partner coordination, and tracking participant milestones
· Provide project support to other NYREACH programs, such as home-delivered meals, food access, and community events
· Coordinate new program launches and pilots from planning through go-live
Cross-Functional Collaboration
· Work closely with Managers, the Compliance & Data Analyst, navigators, and recertification staff to keep projects aligned
· Serve as a main point of contact for project logistics with health plans, subcontractors, vendors, and community partners
· Coordinate meetings, agendas, and follow-ups with internal teams and external partners
· Coordinate with Essen Health Care and other partner organizations on shared projects as needed
Reporting & Documentation
· Prepare weekly project status updates and monthly summaries for the Director and executive leadership
· Work with the Compliance & Data Analyst to make sure project data and dashboards are accurate and current
· Keep project files, meeting notes, and decisions organized in SharePoint and easy for staff to find
· Track contract, data-sharing, and documentation requirements tied to each project so nothing is missed
Key Performance Targets
· All active projects have a named owner, timeline, and status updated at least weekly: 100%
· Project milestones and contract deliverables completed on time: ≥90%
· Risks and delays escalated to leadership before the deadline, not after: 100%
· Weekly status reports delivered on schedule: 100%
